Subject description: Icons won't show up no matter what I do

Was hoping maybe someone around here has a bigger knowledge of Windows than me and could fix this. My icons have been blank for awhile now, for example my firefox folder:

Anyone know what could be the issue, I've looked up and down at the registry, tried just about everything I could find on superuser.com, etc. and everything seems to be just fine, yet the icons still won't work. I can change the icons of things to a second icon in the list and be fine, but it's just the first icon that won't work. Things like Steam that don't have a 2nd icon in it's exe obviously can't be fixed that way either otherwise I would just change the "%1" to "%2" in the registry. I've tried deleting the iconcache, that does nothing, fixed all the file associations; assoc .exe just comes up .exe=exefile. I've repairing the system files, all my explorer and shell files are defaulted now, and still no change.

Any ideas as to the cause?

EDIT: This is x64 Windows 7 Ultimate. RT_ICON seems to be not reading icons at all is one cause and fixed that issue (when asking for file size, was recieving "169478669x109051904, 389715300" expecting "256x256, 32" or "WIDTHxHEIGHT, COLORMODE" -- ;TLDR wanted PNG instead of ICO), problem is still ongoing.

It only happens with .exe files?
If so, it has to be a file association problem, weird.

Maybe some rogue program keeps changing it back?
Did you try booting in safe mode to fix the file assoc?

I didn't think of safe mode for some reason, but I did just do everything I did before, in safe mode. Still having the same issue.
I thought it was a file association problem as well, but everything seems to look perfectly fine as far as that goes.

@deathreaperz - no idea yet #Tongue, but if your problem is any different than mine, maybe the following will work for you:

Hmm. If you aren't scared to lose data, did you look if you had a restore point that precedes the appearence of this problem?
edit: i checked, system restore may only modify "Windows system files, programs, and registry settings". So it is safe for your user data.

If that doesn't resolve your issue, i would reinstall windows imo. :/

Also, you can run the .exe(s) even if they have a blank icon, yes?

Yes, everything works correctly, just the exe's not getting images is the problem. System Restore didn't change it either.

I've been going to reinstall for weeks, but I refuse to let tech outsmart me. #Tongue

EDIT: And after said weeks, I have finally got it fixed. I went to HKEY_CLASSES_ROOT\exefile\shellex\IconHandler\in the registry, deleted the IconHandler default, deleted my iconcache, closed explorer, reran it, repeated that process 2 more times until Windows FINALLY figured out what I wanted it to do, and bam, no more handler and my EXE's have icons. It was so simple, I should have just redone everything several times like everything else you have to do with windows Sad

EDIT2: @deathreaperz - try:
Delete the value in Default in your registry for: HKEY_CLASSES_ROOT\exefile\shellex\IconHandler\
Delete your c:\users\<username>\AppData\Local\IconCache.db.
Kill explorer.exe and run a new instance of explorer.exe to refresh it.

Corrupted icon cache. That happens from time to time.

That's how you know when you got the really good viruses. #Tongue
