Use SHP Builder. Go to Tools, Misc, Cameo Generator.
Or just hit Ctrl+G.

The rest is up to you.

I tried shp builder but the icons look either really small or just plan suck.

You can create an image in photoshop to make your cameo, swap your image to indexed colours and save it as .pcx
